Admissions: Easy and Respectful

At The New Foundation, we recognize the sensitive nature that surrounds the process of seeking and receiving care for mental health and substance abuse issues. So, we strive to make the process as comfortable and seamless as possible.

Admissions Criteria: Making Sure We Can Help

In order to maintain a supportive environment conducive to healing, we must ensure that the TNF care model — a strength-based philosophy combined with our evidenced based treatment approaches– will work for each and every client.

Our determination is based on level of function, willingness to participate in treatment, history, and referring issues.

Getting Started Is Easy:

An admissions coordinator will complete a confidential prescreening assessment to determine if any of The New Foundation’s programs are right for you and your family.

To expedite the admissions process, please submit any paperwork you feel would help us better understand you/your child’s needs.

These include:

  • TNF Referral Form
  • Psychiatric Evaluations (within last 2 years)
  • Psychiatric/Therapy progress notes
  • Behavioral Health Inpatient/Hospital Records
  • Individualized Education Plan (IEP) -if applicable
  • Other clinical records

Please submit via confidential fax to 480-945-9308 or email us.

Once a determination has been made of whether we can meet the needs of your child in treatment we can move towards scheduling the admission and beginning the journey of recovery. To better expedite this process completing a Referral Form is recommended as well as sending us any past clinical records for review. Please see the release of information below to request records from your previous or current providers.

Please see below a list of what to bring at the time of Admission

Clarifying What Costs Are Covered

We understand that parents are often concerned about what the cost of treatment might be. The New Foundation works with Medicaid and most major insurance companies. Coverage will vary from plan to plan, but benefits can be checked quickly and confidentially by calling the TNF admissions department. Our staff will help make the complicated world of managed health care a little easier to understand.

After we are able verify your insurance benefits, we can provide you with an estimate of what your financial responsibility will be prior to admission.

Providers with whom we regularly work:

  • Aetna
  • United Healthcare
  • Cigna
  • Managed Healthnet or Ambetter
  • BCBS
  • Optum
  • Magellan National
  • AHCCCS
  • And Many More
